I come now in my capacity as a member of the Board of Directors of Green Country Habitat for Humanity and as required by Green Country Habitat for Humanity’s Bylaws and Ethics Policy and attests as follows: I have read, understand and agree to be bound by Green Country Habitat for Humanity’s Ethics Policy; Exhibit A, attached hereto and incorporated herein by this reference is, to the best of my knowledge, a full and complete list of each and every one of my “financial” and “institutional” interests, as those terms are defined in the Ethics Policy. I acknowledge my obligation to update this Attestation in the event of material change and at least annually, which I shall do on the first working day of each calendar year, or such other schedule as may be adopted by Green Country Habitat for Humanity’s Board. Along with my participation on the Board of Directors I commit to the following: 1) Actively promote and advocate Green Country Habitat for Humanity’s work within the Tulsa community. 2) Financially support Green Country Habitat for Humanity. 3) Regularly attend Board of Directors’ Meetings. 4) Review the Board Membership Handbook. 5) Follow all Green Country Habitat for Humanity Bylaws and Policies as set forth by Green Country Habitat for Humanity. I acknowledge that the Governance Committee for the Board of Directors will receive reports on my participation with the above commitments and that my lack of participation on any or all of the commitments could result in not being nominated for future terms of service on the Board of Directors.
